Travel - Forms

• WTPR Reimbursement Form is used for tournament, ref fees and other reimbursement requests from WTPR.
• WTPR Fundraising Approval Form is used to request an approval by the Soccer Advisory Board for a team fundraiser.
• NJYS Membership Application is mandatory for player to be included on a team roster
• Volunteer Coaches Membership Form is mandatory for coaches to be included on a team roster
• USYS Medical Release Form is mandatory for player to be included on a team roster
• Player/Coach Status Form is required when adding a player/coach to a roster after the team roster has been approved by the district commissioner. Four copies will print labeled white, yellow, pink, and gold copy.
• Employment/Volunteer Disclosure Statement (KidSafe Form) is required for all head coaches and assistants in Travel/Intramural/Clinic leagues
