Below is a brief description of some of forms that you may need. Information on how to be Megan’s Law certified is also available. All forms are available on the left.

WTPR Reimbursement Form: used for tournament, ref fees and other reimbursement requests from WTPR.

WTPR Fundraising Approval Form: used to request an approval by the Soccer Advisory Board for a team fundraiser.

WT Soccer Trainer Approval Form: Teams using Non-Volunteer paid trainers must use this form to receive approval from the appropriate travel soccer commissioner.

Full Sided Team Budget: used to inform the parents of Full Sided Teams (U11 & up) to the additional costs for the upcoming season

Small Sided Team Budget: used to inform the parents of Small Sided Teams (U8 - U10) to the additional costs for the upcoming season

Employment/Volunteer Disclosure Statement (KidSafe Form): required form for all head coaches and assistants in Travel/Intramural/Clinic leagues

Megan’s Law: required background check for all head coaches, assistant coaches, team managers, trainers, and team moms/dads.

Ratings Sheets: used by coaches for rating Clinic III, Intramural and Travel Players.
